I wrote so that Christians don't just write "Christians" but specify, for example, "Orthodox" or "Catholics".Your question about "church/denomination" makes it appear that it's for Christians and not others including pagans, SPNR and other religions.
Survey design and methodology is a whole field one can study - from the design of your survey it doesn't look like you're attempting to do a proper or professional research survey so I honestly wouldn't worry about it. But if you're interested for curiosity's sake, PEW does a lot of well-grounded research using proper methods and presents it in a way that is accessible to the general public.Thank you for your reply! Personally I would make it two questions. The first Religion (Christian, Buddhist, Pagan etc) The second question would be "Denomination or branch" which I think covers the subgroups for other religions.
