Let's start with basics.

In the beginning (when things had cooled enough) to was hydrogen. After a while two hydrogen atoms got together and a little baby helium atom was born.

I would say that yes, the form of hydrogen atoms made helium atoms.
with or without entropy they made helium

a form is not an action. can an inactive thing create? can a form create itself, or recreate itself without changing its form?

can it defy the law of entropy and remain the same?

A hammer can be swung to insert nails to build a house. In the hands of living people.

I don't get why this question is hard.

Or are they regarding "forms" as objects or concepts, and "things" as living things? In that case, I would say only the reverse is true.

Some is not all, so the form changes with any loss/gain.

Some remained unchanged, we know this because there is considerable amounts of the first element remaining.

Some changed because we can see other, heavier elements now exist

There is no loss or grain, just change

the point is, that things change and in changing the form is not constant, eternal

so a thing, a form is not constant but change, entropy is in forming, creating, and even destruction of a molecule.

A thing can combine with another thing making a different thing.

And the different thing may break apart creating multiple different things

Or the thing may remain unchanged.

If you want to go smaller than a hydrogen atom then you are entering the world of quantum interaction. That is a whole different ball game
