As I've explained before, a poltergeist isn't necessarily a human spirit or a non-human entity. While human spirits and non-human entities can cause a lot of physical and psychological disturbances, that isn't always the case in a poltergeist haunting. Poltergeist activity can also originate from a metaphysical energy associated with a living person who is experiencing a lot of stress in their life, or it could be an unexplained metaphysical energy that is connected to a specific location. If it is a human spirit that is causing the poltergeist activity, then yes, it is possible for that spirit to follow a living person home.
