Strangely, no-one wants to participate in the developed world's most glaring humanitarian disaster. I wonder if it's the thought of rationing $500 vials of insulin, being charged $3000 for an ambulance or $100 for a band-aid, losing your house to pay for cancer treatment, or being one of the 40,000 annual avoidable deaths due to lack of health insurance that's putting people off?
